Registered Nurse – Behavioral Health
$44.00- $52.00 + SHIFT DIFFERENTIAL – NOC

This is a full-time, 36 hour/ week position. Shifts are 12 hours from 6:00pm – 6:30am

Phoenix, Arizona

The registered professional nurse provides, coordinates, and evaluates patient care through collaborative efforts with health team members in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Medical clinical management is conducted in collaboration with other health care team members.

Responsibilities
Assessment and Documentation

  • Assess each assigned patient as needed and per facility policy.
  • Document nursing services provided and the patient’s response to/effectiveness of the services for each assigned patient.
  • Document and reassess any significant changes in the patient’s condition in compliance with facility policy.
  • Report any signs or symptoms of infection in patients according to facility policy. Complete and report Acuities to supervisor for the next shifts staffing levels.
  • Complete and document admission and discharge assessments of assigned patients.
  • Provide medications and documentation per facility policy and procedure.

Treatment Planning:

  • Initiate initial treatment plans and medical problem sheets.
  • Participate in the development of an interdisciplinary treatment plan with specific and measurable goals, objectives and interventions defining actions unique to each patient’s needs.
  • Update and revise the plan as goals/objectives are met or when the patient’s condition changes.
  • Document the nursing interventions on the treatment plan and incorporate them into patient treatment.

Milieu:

  • Assure patients are adequately oriented to the program. Monitor program activity and encourage patient participation.
  • Provide structure and maintain a therapeutic environment in collaboration with the treatment team.
  • Consistently utilize universal precautions and environmental safety guidelines.
  • Utilize self as a role model to promote health communication.

Teaching:

  • Provide education to patients and families based on their identified needs and limitations.
  • Provide and document discharge education including food and drug interactions, diets and activities as appropriate.
  • Evaluate and document the effectiveness of all patient teaching.
  • Provide opportunities for patients and families to ask questions about prescribed therapies and interventions.
  • Provide education for staff on current Healthcare Related Infection risks within the agency.


Performance:

  • Participate in Quality Improvement programs.
  • Collaborate with other team members.
  • Act as the patient’s advocate and assure that patient rights are upheld.
  • Comply with established measures for infection control, OSHA regulations and risk management policies.
  • Use time management skills to accomplish assigned responsibilities timely.
  • Assist co-workers and visitors as needed in a prompt and courteous manner.
  • Follow facility, departmental and personnel policies and procedures.
  • Comply with the employee Code of Ethics.
  • Demonstrate flexibility when assigned to different areas to meet the needs of the hospital.’
  • Attend mandatory education programs and annual in-service training.
  • Complete Online Health Training on or before assigned due dates.
  • Perform other duties as assigned by the supervisor.
  • Model being a team-player, assisting in any function as needed to support the healthcare team and meet patient’s needs

Qualifications

  • Education:
    • Graduate from an accredited program of professional nursing required.
  • Experience:
    • A minimum of three (3) years’ experience in psychiatric and substance abuse preferred. A minimum of one (1) Year medical.
  • Licensure:
    • Currently hold unencumbered licensed to practice by the Arizona State Board of Nursing.
  • Additional Requirements:
    • BLS certification upon hire. Must hold a current Fingerprint Clearance Card or be able to obtain one. Must have Workplace safety such as AVADE or CPI.
  • STANDARDS OF PERFORMANCE:
    • THE ESSENTIAL JOB FUNCTIONS ARE CATEGORIZED UNDER THE JCAHO STANDARDS OF PERFORMANCE and the AZ state board of nursing.
