Discharge Coordinator

The Discharge Coordinator is responsible for facilitating communication between external inpatient/hospital staff and employees. The Discharge Coordinator ensures all appropriate appointments are scheduled post discharge from an inpatient setting, and assisting with clients’ crises as needed.
This is a full-time, 40 hour/ week position. Shifts are Monday – Friday 8-5pm
Responsibilities
- Tracking and coordinating care of clients who are in an inpatient setting for psychiatric care, ensuring that the appropriate outpatient discharge appointments are given within the required timeframe.
- Ongoing coordination with inpatient and outpatient providers to facilitate communication and post-discharge plan of action.
- Facilitates discharge planning while notifying all appropriate authorities and agencies of risk situations, such as Adult Protective Services, Child Protective Services, law enforcement, etc.
- Demonstrates ability to regularly change priorities to accomplish all tasks despite frequent interruptions.
- Demonstrates ability to clearly communicate, both orally and in writing, while performing all essential functions.
- Performs crisis intervention for patients in urgent need of care.
- Conducts behavioral health assessments and recommends psychiatric hospitalizations, as needed.
Qualifications
- Minimum of Masters’ Degree in Counseling or Social Work or related field required.
- Current associate or independent licensure from Arizona Board of Behavioral Health Examiners as either LAC, LPC, LMSW, LCSW, LAMFT, LMFT, LASAC or LISAC preferred
- 3 years’ experience in discharge planning, care coordination, or successfully counseling individuals in crisis situations.
- Experience within a multi-cultural health care environment strongly preferred, with direct patient serving experience preferred.
- Valid Arizona drivers’ license with no major violations during the prior five years.
- Demonstrated ability to relate to diverse cultures and specifically our Community and/or other cultures.
- AZ Department of Public Safety Fingerprint Card within 60 days in position
